Caregiver of the Month Nomination 

Sanders Mortuary is dedicated to recognizing the exceptional efforts of professional caregivers who go above and beyond. We invite you to nominate any deserving individuals who have made a significant impact through their care. Nominees may be hospice caregivers, nurses, aides, activity coordinators, transport staff, volunteers at caregiving facilities, social workers, etc. 

Every month, we will select one exceptional professional caregiver as our Professional Caregiver of the Month. The winner will be notified and celebrated on our Facebook page, as a testament to their invaluable contribution to society.

Each monthly winner will receive a $25 gift card.
There will be an Annual Luncheon to recognize all winners held in May of the following year.
Recognition & Photo will be presented in person and on our Facebook page.

We kindly request that all nominations be submitted by the 25th of each month for consideration for the following month. Please note that any nominations received after the 25th will be considered for the subsequent month's award. Sanders Mortuary staff will deliver the award to the nominee and nominating individual(s) at an agreed-upon location and time.
